Admissions

AfWDA Program is non- discriminatory. Enrollment is open to children ages 2 1/2 weeks to 5 years of age. All children must have proof of up-to-date immunizations and a signed health appraisal by a licensed physician before they may attend. Pre- registration is required with a non-refundable enrollment fee. Enrollment papers may be obtained from the Early Childhood Office. After choosing the appropriate program for your child and paying the enrollment fee the following forms must be completed and returned to the office.

  • Tuition Express Payment Form
  • Child Information Record
  • Student Enrollment Sheet (front and back)
  • Health Appraisal (physician signature on back)
  • Birth Certificate (copy)
  • Parent Agreement
  • Parent Handbook Form
  • Custody Papers (if applicable)
